Developing a Good Individual Support Plan (ISP)

What is the impact of a good Individual Support Plan (ISP) on a person's life? Huge!

In this training, learn:

to use the ISP as a person-centered planning tool
to identify and use supports and services
to develop strong outcome statements for choice and control
ISP development timelines and procedures

Bring a current copy of your ISP to the training.

Registration deadline: the Monday prior to each session.

TIME: Sessions begin at 6 PM.

1. ACCOMMODATIONS

2. ALTERNATE FORMATS (request 2-3 weeks before session)

3. REIMBURSEMENT (mileage, tolls, parking and childcare expenses may be covered)

4. CREDITS (Act 48 - $10 processing fee)

All training participants receive The Gold Book—Understanding the Office of Developmental Programs in Pennsylvania: Intellectual Disabilities and Autism Services—revised and updated.
