MENTORS FOR SELF DETERMINATION presents a Family Connection (ID) System Works

Are you confused and frustrated? Do you wish someone would listen to your concerns and answer your questions? Do you want to better understand the ID System and how it works? If so, come and join this informal discussion to learn about:

• why you should register for ID Services

• what is a PUNS Form and why you need it

• the difference in the two ID Waivers (P/FDS and Consolidated)

• the ISP (Individual Support Plan)

• services available in the ID System

• Participant Directed Services (what is it and is it for you)

NOTE: This training is hosted by the ARC of Schuylkill County.

The Partnership is Funded by the PA Office of Developmental Programs, Department of Public Welfare

The Partnership Trainings are always free!

When registering, ask about accommodations, alternate formats, (request 2-3 weeks before session), reimbursement

(mileage, tolls, parking and childcare expenses may be covered) and/or credits (Act 48 - $10 processing fee).
